Job Summary:
Under the direct supervision of the Chief Operations Officer, the Nurse Case Manager is a registered nurse (RN) responsible for assessing, planning, and coordinating care plans for select patients with complex medical or behavioral health conditions in order to promote quality, cost effective care.

Under the direct supervision of the Chief Operations Officer, the Nurse Case Manager is a registered nurse (RN) responsible for assessing, planning, and coordinating care plans for select patients with complex medical or behavioral health conditions in order to promote quality, cost effective care.
Responsibilities
- Assesses patient needs; develops, monitors, and evaluates care plans and progress.
- Facilitates interdisciplinary approaches across the continuum of care; coordinates services by primary care provider, specialists, mental health professionals and other medical or non-medical providers; arranges resources, including transportation to meet desired outcomes.
- Advocates for needed services and entitlements; obtains additional resources; intervenes in crises; provides personal support.
- Provides patient and provider education.
- Maintains patients' records by creating detailed case notes; logging events and progress in a timely fashion.
- Consistently communicates patients' progress to referring provider.
- Prepares reports by collecting, analyzing, and summarizing treatment and results data and trends; compiling statistics.
- Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.
- Enhances department and organization reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.
- Adhere to organization policy, procedures and the professional code of ethics.
- Other duties as assigned by supervisor
Qualifications and Skills
- Current, unrestricted, California RN license required.
- Minimum two years experience in clinical nursing, UR, case management, or discharge planning within the past five (5) years.
- Experience using electronic medical records is required.
- Bachelor’s degree in Nursing, Social Work or related field preferred.
- Experience and/or knowledge of National Committee for Quality Assurance (NCQA) requirements for Patient Centered Medical Home (PCMH) preferred.
- Advanced certification in Case Management preferred.
- Knowledge of community resources and counseling/social work practices with high risk populations.
- Experience working with persons in crisis, excellent conflict resolution skills.
- Excellent clinical documentation skills.
- Excellent written and verbal communication skills, ability to establish rapport.
- Ability to interact effectively with medical staff, clinic personnel, patients/families, and possess interpersonal skills that reflect a positive attitude and a sense of commitment.
- Excellent decision, problem solving, research and analytical skills.
- Ability to motivate others towards achieving goals.
- Ability to work independently with strong sense of focus, task-oriented, nonjudgmental, open personal qualities, clear sense of boundaries.
