Health Educator

Job Purpose: To perform health assessments and screenings using standardized tools, provide patient education on chronic health conditions such as diabetes and hypertension, tobacco cessation, nutrition and physical activity in individual, telephone, video conferencing, and group setting. Function as a resource for clinic providers and staff. Assess, design, implement and evaluate health education services.

Duties may include, but are not limited to the following:
- Conduct needs assessments, establish priorities, design, implement and evaluate health education programs, protocols and standards. Use patient population data, provider and patient feedback to prioritize health education offerings and design services that will meet the most burning needs first.
- Coordinate closely with providers, case management, quality, administration and the Patient Advocate to assure that the health education program aligns with organizational mission, priorities, needs and existing service lines.
- Assess patient's educational needs on an individual and/or group basis in consultation with members of the care team and apply this assessment to designing educational interventions.
- Screen patients for health risks using standardized tools such as MCHAT, SHA, IHA, PHQ-2, PHQ9 and SBIRT.
- Conduct brief disease management, lifestyle, health risk, tobacco cessation, nutrition and physical activity counseling with patients and family members one-to one.
- Coordinate, facilitate, and evaluate group visits for disease management and prevention and self-care education.
- Provide the scheduling and logistical support necessary to facilitate successful, well organized, cost effective health education services.
- Review, develop and recommend high quality, culturally appropriate written and audio visual health education materials.
- Coordinate health information projects such as informational displays and community health events.
- Prepare reports, proposals, & documentation as assigned to track the effectiveness of the health education program.
Skills/Qualifications:
- Knowledge and experience working with various age groups and high risk populations during moments of crisis. Excellent customer service, non-judgmental professionalism expected at all times.
- Excellent communication, organizational and time management skills with the ability to meet tight timeframes.
- Ability to work well independently, learn quickly and retain complex information.
- Ability to work effectively and collaboratively with a variety of customers including clinic staff, health plans, patients and physicians.
- Creative problem-solving skills and ability to exercise sound judgment in quickly evaluating situations and problem solving, critical thinking and good clinical judgment. Must be able to maintain appropriate boundaries with clients and professional demeanor.
- Knowledge of relevant healthcare regulations (including HIPAA), accreditation standards and regulatory requirements.
